研究興趣:

 穿顱超音波刺激治療神經退化性疾病以失智症為例

世界各國人口老化趨勢嚴重,失智症風暴席捲全球。失智症是一種疾病,可分為退化性與血管性兩大類,其中最常見的是阿茲海默症,大約佔老人失智五成的比例,是一種持續性神經功能障礙,病患會逐漸喪失記憶,並且有語言和情緒上的障礙。另一為血管性失智症,約佔三成,是因腦中風或慢性腦血管病變,導致腦部血液循環不良,造成腦細胞死亡與智力衰退,為引發失智症的第二大原因。

2015年8月國際失智症協會(World Dementia Council)發布的全球失智症報告,全球失智症人口約4680萬人,一年平均新增990萬名病患,平均每3秒就有1人。預估到2030年,全球失智人數將增至7470萬人;2050年失智症人數恐將高達1億3150萬人。 快速走向超高齡社會的台灣,同樣深陷失智風暴中。台灣失智症協會推估目前每年台灣每天約新增25例失智症者,至130年預計將達到最高峰、每天約新增63名(即該年新增約達23000人)。未來政府及民間都應積極準備迎接失智盛行世代的來臨。

目前並無有效根治失智症的方法,臨床上主要以藥物減緩其惡化;常見者為乙醯膽醶抑制劑及阻斷神經細胞損傷的NMDA受體拮抗劑這兩大類藥品。全球新藥研發的速度推陳出新,已有幾種失智症的新藥正在臨床實驗中。然而,失智症患者被診斷時,腦部功能通常都已損害很多年,即使新藥有療效,已發病的病人也不可能被新藥治癒且藥物常伴隨著副作用。近年來,非藥物治療法逐漸興起,神經學家開始應用許多外在電、磁或光刺激模式來調控中樞神經之興奮性,應用於失智症相關疾病研究,但是其作用機制仍多屬未知。最近,本人在陽明大學所組成治療用超音波研究團隊,在腦部治療用超音波領域研究多年後,有了嶄新突破性的失智症治療技術開發!

近年由本人所帶領的神經退化性疾病研究團隊經過累積多年治療用超音波應用於腦部疾病治療之研究經驗,全球首創利用能夠穿過頭骨且適當參數的低強度脈衝式超音波刺激腦部,使其增加內生性的腦源性神經滋養因子(brain-derived neurotrophic factor, BDNF),以達到預防與治療失智症之目的。相較於藥物治療能夠免除其藥物的副作用,提供安全且創新之腦部神經退化性疾病之非侵入式物理療法,目前針對鋁誘導阿茲海默症與缺血性失智症動物模型進行臨床前動物實驗,結果證實超音波刺激不但可以恢復失智症實驗鼠的學習記憶功能,並且減少實驗鼠腦部神經細胞的死亡。

超音波目標藥物傳輸非侵入式局部開啟血腦障壁

治療用超音波(Therapeutic Ultrasound)目前已成為一個新興潛力的研究領域,其原理是利用非侵入方式將高強度聚焦超音波(High Intensity Focused Ultrasound)能量導入所需治療之區域,利用組織對機械波能量之吸收造成溫昇,將該區域之溫度提昇到43℃~45℃之間或利用高強度機械波壓力產生空穴化作用,產生高溫及高壓對組織進行破壞等現象來去除或治療腫瘤。

此外,前人的研究指出聚焦式超音波結合超音波顯影劑能以非侵入方式將特定區域的血腦屏障暫時開啟。本實驗室研究是探討超音波顯影劑濃度與超音波聲壓力對局部開啟血腦屏障後治療效果與藥物動力學分析。由Evans blue在腦組織的滲出量得知,在相同的聲壓力下高濃度的顯影劑可造成較高的Evans blue的滲出量,而當顯影劑濃度相同時,較高的聲壓力也可以得到類似的結果。此外,在高濃度的顯影劑下,可以使血腦屏障的開啟較集中於焦點區域。從組織切片觀察得知,在適當的低濃度顯影劑時,可以在不傷害正常腦組織的情況下將血腦屏障開啟,然而在高濃度時會造成細胞凋亡的現象而其凋亡數量隨著距離焦點位置的增加而遞減。此研究驗證適當的顯影劑濃度能在不傷害正常腦組織的情況下增強穿顱式超音波將血腦屏障開啟的效率並且更集中於焦點區域。此種超音波藥物目標區域傳輸系統的原理是超音波的機械波與打入病患血管內的微氣泡相互作用所產生的空穴化效應,促使在超音波施打區域的血管通透性增加,達到只有在病灶區域的藥物濃度提昇,進一步提高治療效果。同時,因為能降低病患治療藥物的施打劑量,相對減少了藥物引發其他副作用的可能性。研究開發以磁振造影導引暨監控聚焦超音波開啟血腦障壁與藥物釋放系統,以超音波將治療藥物導引至局部的治療目標區域並增強病灶區域的藥物傳輸量,提高治療效果與治癒率並且以核醫影像做藥物動力學分析。

目前本實驗室已開發的「超音波藥物傳輸監測裝置」技術,適用於各種組織器官。其原理是以都卜勒超音波量測脈衝聚焦式超音波施打血管位置前後之血流速度,除了可以確認施打位置的正確性外,更可由計算而得之收縮期峰值血流速度(peak systolic velocity, PSV) 和搏動指數(pulsatility index, PI)來間接即時監測超音波施打目標區域的藥物滲透量。此關鍵技術及監控軟體可相容於現有各大廠牌超音波系統,未來市場規模潛力極大。另外,將超音波熱療結合放射治療系統之開發亦為本實驗室之研究方向。
